An inquiry form has one job: help a real person start a useful conversation. It does not need to collect every detail before anyone has replied.
For a small business, begin with the basics: name, email, business name, and the question the person wants answered. Add one structured choice that helps route the inquiry, such as “What are you looking for?” Keep optional context separate from required fields so a visitor can finish the form without writing an essay.
Explain how you will use the information and how you will respond. Our Manifestic inquiry form asks visitors to explicitly agree to an email response before submitting.
